Your Personal Data and Special Categories of Personal Data, as specified below, will be processed by Dr. Birgül ALTUNTÜRK in accordance with the fundamental principles foreseen in Article 4 of the KVKK (Personal Data Protection Law) and Article 5 of the GDPR (General Data Protection Regulation), in line with the Personal Data Processing Conditions and Purposes specified in Articles 5 and 6 of the KVKK and Articles 6 and 9 of the GDPR, and within the scope of the purposes stated in this Disclosure Statement; in a lawful manner, in a manner that is relevant to the processing purpose, limited and proportionate, observing the principles of justice and transparency, accurate and up-to-date, and by taking all technical and administrative measures foreseen in the legislation to ensure data security.
1- YOUR IDENTITY INFORMATION: Your name, surname, Turkish Republic Identity Number and/or Passport Number and/or Temporary Turkish Republic Identity Number 1. Your Identity Number: Your place and date of birth, marital status, gender, occupation, signature, and other identifying data.
2. Your Contact Information: Your address (residence/workplace), telephone number (your registered home/workplace landline and/or mobile phone numbers), email address, IP address, social media accounts, and other contact information.
3. Your Health Information: Your blood type, allergies, chronic diseases, sexually transmitted diseases, infectious diseases, data regarding previous surgeries/operations, medications you regularly use, information regarding Covid-19, medical treatments, health reports, test and imaging results, prescription information, body analysis and measurement data, skin analysis data, harmful habits, and other health data necessary for treatment and medical procedures.
4- PHOTOGRAPHS AND VIDEO IMAGES: Photographs and/or videos taken before, during, and/or after medical procedures.
5- FINANCIAL INFORMATION: Your bank account number, IBAN number, credit card information, billing and invoice information, and other financial data.
6- TRANSFER AND ACCOMMODATION INFORMATION: If you are a Health Tourist coming under the scope of International Health Tourism, your transfer information, round-trip flight ticket information, and hotel accommodation information.
Your Personal Data; To create patient files, conduct examinations, preventive medicine, medical diagnosis, treatment, and care services, perform follow-up checks after medical diagnosis and treatment processes, manage potential complications, communicate directly with you, manage appointment processes, manage patient satisfaction and demands, fulfill legal and contractual obligations, maintain your health data as required by relevant legislation for specified periods, obtain consultation services from other relevant specialists when necessary to ensure your treatment is carried out correctly, fulfill legal obligations in accordance with the legislation within the scope of International Health Tourism, plan transfer and accommodation services for patients/clients coming within the framework of Health Tourism, announce innovations related to medical treatments and practices, inform third parties about the medical procedure performed, conduct promotional activities for medical services implemented within the framework of Health Tourism Incentive Legislation, plan and manage health services and their financing, fulfill responsibilities arising from the legal relationship established between doctor and patient, fulfill financial and administrative obligations, and ensure technical and commercial security. Your personal data is processed by Dr. Birgül ALTUNTÜRK in accordance with the KVKK (Personal Data Protection Law) and GDPR (General Data Protection Regulation) for the purpose of fulfilling public obligations.
Your Personal Data will be processed by Dr. Birgül ALTUNTÜRK under the following conditions and circumstances:
Your Identity Data will be processed to carry out examination, preventive medicine, medical diagnosis, treatment and care services, to create patient files, to manage patient satisfaction and demands, and to plan transfer and accommodation services for patients/clients coming within the framework of Health Tourism.
Your Contact Information will be processed to conduct your follow-up checks after medical diagnosis and treatment processes, to manage appointment processes, and, with your explicit consent, to introduce, inform, and announce innovations related to medical treatments and applications, to remind you of your appointment dates for ongoing treatments, and to send congratulations and greetings on special occasions.
Your Health Information; Your personal data will be processed in order to successfully carry out your medical treatments, to obtain a consultation from another specialist physician if deemed necessary during your treatment process, to create a patient file, to retain information regarding your health data that must be kept in accordance with the relevant legislation for the prescribed period, and to fulfill legal obligations under the current Health Law and Health Tourism Legislation.
Your photographs, videos, and audio recordings will be processed by Dr. Birgül ALTUNTÜRK to monitor and manage your treatment process. With your explicit consent, they may also be processed to inform and promote the medical procedures you undergo to third-party health tourists wishing to receive treatment in our country within the scope of International Health Tourism.
Your financial data will be processed to control your payments, issue invoices, and process refunds in case of any excess charges.
Your transfer and accommodation information will be processed to plan transfer and accommodation services for patients/clients arriving within the framework of International Health Tourism.
The above-mentioned Personal Data and Special Categories of Personal Data will be processed in accordance with the purposes and legislation mentioned above, in order to carry out your medical treatment and fulfill the obligations related to your treatment. Failure to provide this personal data will prevent Dr. Birgül ALTUNTÜRK, who will be providing medical treatment, from fulfilling her legal obligations properly, and your treatment and/or recovery processes may not be successfully completed.

and, upon your request and when necessary, via written/audio/visual (photographs and/or video recordings) messages and online audio/video calls you make through remote connection application service providers (whatsapp/zoom.us/facetime/skype/messenger/google/instagram/facebook, etc.) from whom you receive services by accepting their Privacy Policies and International Transfer Policies, for the purpose of enabling Dr. Birgül ALTUNTÜRK to conduct online diagnoses and check-ups remotely,
and, by sharing your personal information on social media accounts (instagram, youtube, facebook, etc.) whose servers are located abroad, of which you are currently a user, by accepting their Privacy Policies and International Transfer Policies. By sending a direct message to Dr. Birgül ALTUNTÜRK’s profile account (Twitter, LinkedIn, etc.) and/or commenting on her posts,
By accepting their Privacy Policies and International Transfer Principles, you consent to the automatic processing of your information through panels such as “contact us” or “get information” in the advertisements and promotions made by Dr. Birgül ALTUNTÜRK on social media accounts (Instagram, YouTube, Facebook, Twitter, LinkedIn, Google, etc.) whose servers are located abroad, which you are currently a user of, and through which you transmit this information;
Through photographic/video recordings taken before, during, and/or after medical procedures performed by Dr. Birgül ALTUNTÜRK for the purpose of observing your treatment;
Through log-recording devices (firewalls) used within the scope of technical measures taken in accordance with regulations, if you wish to join the wireless internet network (Wi-Fi) available in Dr. Birgül ALTUNTÜRK’s clinic as a guest user, data will be collected and processed throughout the examination, preventive medicine, medical diagnosis, treatment, and care services provided by Dr. Birgül ALTUNTÜRK. Personal data collected from Birgül ALTUNTÜRK will be processed in accordance with the KVKK (Law on Protection of Personal Data) and GDPR (General Data Protection Regulation) in order to enable her to fully and properly fulfill her contractual and legal obligations.
4. Transfer of Personal Data
Your Personal Data, collected in accordance with the conditions and purposes set forth in Articles 5 and 6 of the KVKK No. 6698 and Articles 6 and 9 of the GDPR, will be transferred to Dr. Birgül ALTUNTÜRK in accordance with Articles 8 and 9 of the KVKK and Articles 45 and 49 of the GDPR. Your Personal Data and Special Categories of Personal Data processed by Dr. Birgül ALTUNTÜRK may be transferred to the following third parties and institutions, to the extent sufficient for the achievement of the purpose, by signing necessary confidentiality agreements and ensuring all administrative and technical security measures in accordance with the legislation, for the purposes of conducting and developing examination, preventive medicine, medical diagnosis, treatment and care services, managing complication processes, obtaining consultation services from other specialist physicians when necessary, fulfilling administrative obligations related to International Health Tourism legislation, planning transfer and accommodation services for patients arriving within the framework of Health Tourism, managing promotional activities in accordance with International Health Tourism Incentive legislation, communicating with patients, planning and managing health services and their financing, fulfilling responsibilities arising from the legal relationship established between doctor and patient, fulfilling financial, legal and administrative obligations, ensuring technical and commercial security and fulfilling public obligations. In this context, your Personal Data and Special Categories of Personal Data processed by Dr. Birgül ALTUNTÜRK;

5. Data Subject Rights
Individuals whose personal data is processed by Dr. Birgül ALTUNTÜRK have the following rights pursuant to Article 11 of the Law:
• To learn whether their personal data is being processed,
• To request information regarding the processing of their personal data, if it has been processed,
• To learn the purpose of the processing of their personal data and whether it is being used in accordance with its purpose,
• To know the third parties to whom their personal data has been transferred, domestically or internationally,
• To request the correction of their personal data if it is incomplete or inaccurate, and to request that this action be notified to the third parties to whom their personal data has been transferred,
• To request the deletion or destruction of their personal data if the reasons requiring its processing have ceased to exist, even if it has been processed in accordance with the KVKK (Personal Data Protection Law) and other relevant laws, and to request that this action be notified to the third parties to whom their personal data has been transferred,
• To object to a result that is detrimental to them arising from the analysis of their processed data exclusively through automated systems,
• To demand compensation for damages incurred as a result of the unlawful processing of their personal data.
The data subject may submit their requests regarding these rights to the Data Controller in accordance with the Communiqué on the Procedures and Principles for Applications. To exercise your rights mentioned above, you may personally deliver your request along with the necessary information identifying your identity, through a notary public, or through other methods determined by the Personal Data Protection Board.
The data subject’s requests will be evaluated and decided upon free of charge as soon as possible, and ultimately within thirty (30) days at the latest. If the evaluation and decision-making process requires additional costs, the fee will be based on the tariff determined by the Personal Data Protection Board.
